About The Position

Inspired Home Health is seeking a Physical Therapist to provide high-quality, individualized home health therapy to clients in the Cave Creek area. The role supports safe mobility, functional independence, and measurable recovery through personalized plans of care. The workday is organized around scheduled in-home appointments. The therapist reviews notes and care plans, travels between patients’ homes to provide one-on-one therapy, updates documentation, confirms appointments, and coordinates with the clinical team as needed. At the end of the day, the therapist closes charts, prepares for the next day’s caseload, and ensures patients understand their next steps for continued progress. Mileage reimbursement is provided.

Requirements

Current Arizona Physical Therapist license or eligibility for licensure.
Strong clinical reasoning and patient-centered communication.
Reliable transportation for in-home visits in Cave Creek.
Ability to document accurately and meet visit timelines.
Hands-on CPR certification.
TB test results from within the last year.
Must pass the OIG exclusion list check and MVR check.
Arizona fingerprint clearance card required.

Responsibilities

Complete patient evaluations.
Develop and update treatment plans.

Coordinate care with physicians and the care team.
Educate patients and caregivers about exercises, safety, and progression.
Review notes and care plans before appointments and confirm upcoming appointments.
Travel between homes to provide one-on-one therapy.
Close out charts and prepare for the next day’s caseload.
Ensure patients have clear next steps for continued progress.
