About The Position
Emerald Care Center Midwest is seeking a skilled, motivated Physical Therapist to provide rehabilitative care to residents in a skilled nursing environment. The Physical Therapist assesses residents and develops, implements, and adjusts individualized therapy programs to promote functional independence, recovery, and overall well-being. The role involves collaborating with a multidisciplinary healthcare team and educating residents and their families. This is an as-needed per diem position with flexible scheduling in a clinical healthcare environment.
Requirements
- Doctoral or Master’s degree in Physical Therapy from an accredited institution.
- Current state licensure as a Physical Therapist.
- Strong clinical assessment and treatment skills.
- Effective communication skills.
- Ability to work as part of a collaborative healthcare team.
- Compassionate, patient-focused approach and commitment to high-quality care.
- Strong organizational skills and attention to detail.

Responsibilities
- Evaluate patients’ physical conditions, mobility, and functional abilities and develop individualized treatment plans.
- Create and implement therapy programs to address patient goals, improve mobility, alleviate pain, and enhance physical function.
- Administer therapeutic exercises, manual therapy techniques, and modalities.
- Assess and document patient progress, adjusting treatment plans as needed.
- Educate patients and families about exercises, safety measures, and home care strategies.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and address issues affecting patient progress.
- Maintain accurate, timely documentation of evaluations, treatment plans, progress notes, and discharge summaries in accordance with facility policies and regulatory requirements.
- Follow safety and infection control protocols and facility policies and procedures.
