About The Position

Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Maintain thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and the standards of practice of MGH Home Care. The role requires travel to assigned cases.

Requirements

• Massachusetts Physical Therapist professional license.
• One year of traveler experience.
• One year of home care experience.
• Ability to travel to cases as assigned.

Responsibilities

• Provide physical therapy services to patients in their place of residence.
• Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs.
