About The Position

Focused Post Acute Care Partners (FPACP) is seeking a Licensed Physical Therapist to join their team at Focused Care at Sherman in Sherman, TX. The Physical Therapist will be responsible for evaluating patients, developing and implementing physical therapy treatment programs in collaboration with physicians, and administering therapeutic exercises and modalities. The role also involves instructing patients and caregivers, and supervising Physical Therapy Assistants.

Requirements

Bachelor's Degree, Master's Degree, or Doctorate Degree in Physical Therapy from an accredited college or university.
Current license/registration by state of hire as a Physical Therapist.
Able to lift up to 50 pounds.
Able to perform one-person transfer under routine and emergency situations.
Able to reposition a bedbound patient alone.
Able to sit, stand, bend stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.

Responsibilities

Evaluate patients and develop physical therapy treatment programs in collaboration with the physician.
Provide physical therapy treatments to patients.
Administer complex active and passive therapeutic exercises, therapeutic massages, and various modalities.
Instruct, motivate, and assist patients/caregivers in learning and improving activities of daily living.
Supervise and direct the efforts of the Physical Therapy Assistant, if one is assigned.
Establish, implement, and evaluate treatment programs, plans of care, goals, and discharge plans for patients related to Physical Therapy needs, in conjunction with the physician.
Revise plans and goals based on changes in patient status.
Provide direct patient care services to ensure the effectiveness of Physical Therapy programs.
Apply heat, light, water, and electrically based treatments/modalities (e.g., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy) as prescribed.
Prioritize, plan, and manage treatment and travel times for maximum efficiency while meeting patient needs.
Ensure orders are carried out professionally, satisfying professional ethics, patient rights, and State and Federal codes and regulations.
Facilitate effective communication among other clinicians providing care to the patient.
Assist in coordinating the Physical Therapy program with other appropriate health care providers/disciplines.
Participate in interdisciplinary patient care conferences and maintain ongoing communication with all disciplines involved in the patient Plan of Care.
