About The Position

Focused Post Acute Care Partners (FPACP) is seeking a dynamic RN Supervisor to join their team in Sherman, TX. This role involves assisting the Director of Nursing in supervising nursing services and setting care standards to meet residents' medical, physical, emotional, and psychological needs. FPACP emphasizes a culture of excellence and has a recognition program called ROCKStar (Recognizing Outstanding Care and Kindness).

Requirements

A registered nurse licensed and in good standing with the Texas State Board of Nurse Examiners in the state of Texas.

Responsibilities

Supports the Director of Nursing in monitoring, evaluating, and organizing direct nursing care.
Effectively communicates with the DON, MD, team members, and residents to promote clinical outcomes and continuity of care.
Works cooperatively as part of the interdisciplinary team to assess, develop, implement, and evaluate resident care plans.
Makes frequent rounds throughout the shift and assists residents and staff as needed.
Uses critical thinking for resident conditions and implements interventions to prevent avoidable rehospitalizations.
Monitors completion of electronic medical records/documentation.
Educates staff on policies and processes when necessary.
Supports newly hired nursing staff.
