About The Position

Focused Post Acute Care Partners (FPACP) is seeking an RN Supervisor to join their team at Focused Care at Webster in Webster, TX. The RN Supervisor, under the direction of the Director of Nursing, will assist in supervising nursing services and setting standards of care to ensure a professional, skilled program that addresses each resident's medical, physical, emotional, and psychological needs. FPACP emphasizes a culture of recognition through its ROCKin recognition program, ROCKStar, which offers a gamified experience for team members to level up and potentially attend an annual gala.

Requirements

A registered nurse that is licensed and in good standing with the Texas State Board of Nurse Examiners in the state of Texas.

Responsibilities

Supports the Director of Nursing in monitoring, evaluating, and organizing direct nursing care.
Effectively communicates with the DON, MD, team members, and residents to promote the best clinical outcomes and enhance continuity of care.
Works cooperatively as part of the interdisciplinary team to assess, develop, implement, and evaluate resident care plans.
Makes frequent rounds throughout the shift and assists residents and staff when necessary.
Uses critical thinking for resident conditions and implements interventions to prevent avoidable rehospitalizations.
Monitors completion of electronic medical records/documentation.
Educates staff on policies and processes when necessary.
Supports newly hired nursing staff.
