About The Position
FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Reseda and the surrounding areas. As an independent contractor, you will have the flexibility to control your own schedule and work in your preferred coverage areas. You will be your own boss and can take as much time off as you want. FeldCare Connects provides administrative support including assigning, communication, scheduling, care coordination, and quality assurance. You will observe, record, and report the patient's response to treatment and changes to the patient's condition, and determine the plan of care. The Electronic Medical Record system is intuitive, making documentation easy, with many clinicians completing documentation during the visit. You will join a group that shares your passion for helping people and have access to patients through an innovative app, Cliniconnects, to accept or decline patients in seconds.
Requirements
- Current Physical Therapist license issued by the state board of Physical Therapy.
- The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
- Completion of an accredited Physical Therapy program.
- Reliable personal transportation.
- Smartphone.

Responsibilities
