About The Position

Boost Home Healthcare is hiring a part-time, field-based home health Physical Therapist serving patients across Greater Boston. The role involves intermittent visits to patients’ residences to provide physical therapy that helps them regain strength, mobility, function, and independence. The Physical Therapist follows physician orders and the plan of care under the direction of the Director of Clinical Services or Clinical Supervisor and works with a care team that provides field support. The position is listed at $60,000–$65,000 per year and includes flexible work-from-home options.

Requirements

Degree in physical therapy from a CAPTE-accredited program.
Current Massachusetts PT license.
Working knowledge of Medicare and OASIS documentation.
Comfort working independently in the field.
Reliable transportation for travel between patient homes.

Two or more years of home health experience is preferred.

Responsibilities

Travel to patients’ homes to perform initial evaluations and develop individualized plans of care.
Deliver physician-ordered physical therapy to restore function, mobility, and independence.
Teach patients and caregivers home exercise programs and home safety strategies.
Reassess patients at least every 30 days and update the plan of care.
Complete timely, accurate Medicare OASIS and visit documentation.
Coordinate care with nurses, therapists, social workers, and physicians.
Supervise therapy assistants as required.
