About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in the greater West Palm Beach, Florida area. Clinicians deliver care in private residences and Senior Living Communities, including Independent Living, Assisted Living, and Memory Care, using FOX’s Geriatric House Calls™ model. The role focuses on improving patients’ mobility, independence, wellness, longevity, and quality of life through evaluations, individualized plans of care, and evidence-based treatment.

Physical Therapists can create and manage their own treatment schedules and develop lasting patient relationships. Telehealth and other virtual care may also be incorporated when appropriate. Clinicians collaborate with an interdisciplinary team and educate patients, caregivers, and senior living staff. Available employment pathways include full-time, part-time, and PRN / FOX Flex roles, with compensation and benefits varying by employment type and eligibility. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program. Qualified clinicians may be eligible for H1B sponsorship.

Requirements

• Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living settings, including Independent Living, Assisted Living, and Memory Care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Travel by driving to patient homes and senior living communities.
