About The Position

Aspire Home Healthcare is seeking a part-time Physical Therapist for 2-3 days a week in the Chico/Oroville area. This is a paid per visit position. Aspire Home Healthcare provides patients with around the clock care and support. We offer a competitive salary, flexible scheduling, mileage reimbursement, 3 weeks of PDO per year, one-on-one patient care, referral bonuses up to $500, and regular evaluations with annual merit increases.

Requirements

  • Licensed PT in CA.
  • Physical within 6 months prior to first patient contact.
  • Current TB results (within last 12 months) and CPR Certification.
  • Valid driver's license and proof of liability insurance on automobile.
  • Ability to attend 1-day orientation at our Yuba City office.

Nice To Haves

  • Minimum of 1 year as an PT experience preferred.
  • OASIS and Wellsky (Kinser) experience strongly preferred.

Responsibilities

  • Interprets and evaluates recommendations and orders.
  • Assesses assigned patients' capabilities and disabilities.
  • Plans and carries out a progressive treatment program.
  • Coordinates treatment program with the total rehabilitation program.
  • Provides effective patient care using approved treatment techniques and procedures.
  • Evaluates patient's condition, functioning level, and treatment goals, keeping current records.
  • Plans, administers, and supervises PTAs and HHAs in accordance with the treatment plan after evaluating the patient.
  • Develops treatment plans for each patient and performs appropriate individual treatment.
  • Documents patient's treatment in the medical chart daily.
  • Provides therapy services as outlined in the Plan of Care.
  • Assists patient and family with the development and review of home programs.
  • Evaluates and reviews adaptive equipment needed in the home setting.
  • Evaluates the onsite home setting when necessary.
  • Charts services rendered, notes changes in patient's condition and/or family and home situation, makes revisions in the care plan as needed.
  • Records supervisory visits, evaluates patient care and progress.
  • Closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
  • Maintains confidentiality of client's situation, records, and services provided according to HIPPA regulations.
  • Evaluates the effectiveness of therapy services to the individual and family.
  • Submits all patient notes and/or visit records on a timely basis.
  • Obtains orders for paramedical services and submits referrals to the Director of Clinical Services (e.g., nursing services, medical social services).
  • Coordinates total patient care.
