About The Position

Aspire Home Healthcare is seeking a part-time Occupational Therapist for their Northern California branch. This is a paid per visit position providing patients with around-the-clock care and support. The OT will interpret and evaluate recommendations and orders, assess patients' capabilities and disabilities, plan and carry out progressive treatment programs, and coordinate treatment with the overall rehabilitation program.

Requirements

  • Licensed OT in CA.
  • Physical within 6 months prior to first patient contact.
  • Current TB results (within last 12 months) and CPR Certification.
  • Valid driver's license and proof of liability insurance on automobile.
  • Ability to attend a 1-day orientation at our Yuba City office.

Nice To Haves

  • Minimum of 1 year as an OT experience preferred.
  • OASIS and Wellsky (Kinser) experience strongly preferred.

Responsibilities

  • Provide effective patient care using approved treatment techniques and procedures.
  • Evaluate patient's condition, functioning level, and treatment goals, maintaining current records.
  • Plan, administer, and supervise COTAs and HHAs in accordance with the treatment plan after patient evaluation.
  • Administer appropriate evaluations to assess patient's level of function and develop therapy goals.
  • Develop treatment plans for each patient and perform appropriate individual treatment.
  • Document patient's treatment in the medical chart daily.
  • Provide therapy services as outlined in the Plan of Care.
  • Assist patient and family with developing and reviewing home programs, evaluating and reviewing adaptive equipment needs, and evaluating the home setting when necessary.
  • Chart services rendered, note changes in patient's condition/family/home situation, revise care plans as needed, record supervisory visits, evaluate patient care and progress, and close charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
  • Maintain confidentiality of client's situation, records, and services provided according to HIPPA regulations.
  • Evaluate the effectiveness of therapy services to the individual and family.
  • Submit all patient notes and/or visit records on a timely basis.
  • Obtain orders for paramedical services and submit referrals to the Director of Clinical Services (e.g., nursing services, medical social services).
  • Coordinate total patient care.
