About The Position
Aspire Home Healthcare is seeking passionate and skilled Registered Nurses (RNs) to join their team and make a difference every day. The RN is responsible for providing quality, one-on-one care that helps patients live comfortably and safely in their homes. The RN will provide direct patient care and case management in accordance with physician orders, including conducting patient admissions, developing care plans, and coordinating care with the interdisciplinary team.
Requirements
- Active RN license in California
- Current physical exam (within last 6 months)
- Current TB test (within last 12 months) and CPR certification
- Valid driver’s license
Nice To Haves
- Experience with OASIS and WellSky (Kinnser) strongly preferred
- Med-surg and wound care experience a plus
Responsibilities
- Obtain medical history and assess patient condition and needs
- Conduct physical assessments, including vitals and mobility evaluations
- Educate patients and families on care plans and home health services
- Deliver skilled nursing care while maintaining HIPAA compliance
- Document and submit accurate visit records promptly
- Develop and update individualized care plans as needed
- Coordinate with LPNs, and Home Health Aides
- Facilitate referrals for additional services such as therapy or medical social work
- Evaluate the effectiveness of nursing services and adjust as appropriate
