About The Position

Join BrightStar Care of Kingwood/Humble as a licensed Physical Therapist providing personalized, one-on-one care in patients' homes. Requirements

  • Master's Degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy program
  • Current Physical Therapist license in the state where services are provided
  • Successful completion of the National Physical Therapy Examination (NPTE) and any state-required licensing examinations
  • Minimum of two (2) years of experience as a Physical Therapist
  • Commitment to maintaining HIPAA compliance and patient confidentiality

Nice To Haves

  • Home health or community-based experience preferred
  • Strong clinical assessment, communication, and documentation skills

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on physician orders and clinical findings
  • Collaborate with physicians and the care team to establish treatment goals appropriate for the patient's home environment
  • Assess strength, mobility, gait, range of motion, balance, pain levels, and functional limitations
  • Recommend and train patients, family members, and caregivers on the proper use of assistive devices and adaptive equipment
  • Implement therapeutic interventions designed to improve function, reduce pain, and enhance independence
  • Identify fall risks and recommend home safety modifications and preventive strategies
  • Monitor patient progress and modify treatment plans as needed
  • Communicate changes in patient condition and treatment response to the physician and Director of Nursing
  • Educate family members and caregivers on exercises, therapy techniques, and ongoing care needs
  • Complete accurate clinical documentation, progress reports, and discharge summaries in a timely manner
  • Treat clients, families, caregivers, and team members with professionalism, courtesy, and respect
