About The Position

Requirements

  • Master's Degree or Doctorate in Occupational Therapy from an accredited occupational therapy program.
  • Current Occupational Therapist license in the state where services are provided.
  • Successful completion of all state-required licensing requirements.
  • Minimum of two (2) years of experience as an Occupational Therapist.
  • Strong clinical assessment, communication, and documentation skills.
  • Commitment to maintaining HIPAA compliance and patient confidentiality.

Nice To Haves

  • Home health or community-based experience preferred.

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on physician orders and clinical findings.
  • Assess patients' ability to perform activities of daily living, including dressing, bathing, grooming, meal preparation, and household tasks.
  • Develop therapeutic interventions to improve functional mobility, coordination, strength, cognition, and independence.
  • Recommend adaptive equipment, assistive devices, and environmental modifications to promote safety and independence.
  • Educate patients, family members, and caregivers on therapeutic techniques, home safety, and proper use of adaptive equipment.
  • Identify safety concerns and fall risks within the home environment and recommend appropriate interventions.
  • Monitor patient progress and adjust treatment plans as needed to achieve established goals.
  • Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care.
  • Communicate changes in patient condition and treatment response to the physician and Director of Nursing.
  • Complete accurate clinical documentation, progress reports, and discharge summaries in a timely manner.
  • Treat clients, families, caregivers, and team members with professionalism, courtesy, and respect.
