About The Position
The Visiting Nurse Association of Florida (VNA) is a nonprofit home health agency with nearly 50 years of service to Florida's communities. As we continue to grow, we are looking for dedicated and compassionate Physical Therapists (PTs) to join our expanding team across the state. This role involves conducting initial patient assessments in the home setting, developing and implementing individualized treatment plans, coordinating care with physicians and healthcare teams, and promoting mobility, independence, and healing through compassionate, hands-on care. You will use our proven case management model to guide patient outcomes and support families.
Requirements
- Active Florida Physical Therapist license
- Minimum 2 years of post-graduate experience preferred
Nice To Haves
- Orthopedic or home health experience is a plus
- Passion for patient-centered care and commitment to quality outcomes
