About The Position

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Clemson, South Carolina, and surrounding communities. Care is delivered in patients’ private residences and senior living settings, including independent living, assisted living, and memory care. Clinicians help patients improve mobility, independence, wellness, longevity, and quality of life through FOX’s Geriatric House Calls™ model.

The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work across in-home and senior living settings. Clinicians may also incorporate telehealth and other virtual care when appropriate. FOX describes its practice as clinician-operated, emphasizing evidence-based care, meaningful relationships, clinician-driven decisions, and a mission to abolish ageism.

FOX lists full-time, part-time, and PRN/Fox Flex employment pathways. Compensation varies by experience and employment type. Eligible roles include benefits, and additional earning opportunities may be available through referrals, clinical instruction, mentoring, supervising assistants, care above quota, and content creation.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to wear PPE when required.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
• Document care in FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
