Fort Worth Physical Therapy, a H2 Health company, is seeking a Physical Therapist to join our team in Grapevine, Texas. At Fort Worth Physical Therapy Grapevine, you will work with a diverse outpatient population, providing individualized, evidence-based care for patients with orthopedic, musculoskeletal, sports-related, balance, dizziness, and post-surgical conditions. Requirements

  • Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.
  • Current Texas Physical Therapist license or eligibility for licensure.
  • CPR certification or willingness to obtain as required.

Responsibilities

  • Perform comprehensive patient evaluations and develop individualized plans of care.
  • Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
  • Treat a variety of orthopedic, post-operative, sports medicine, vestibular, balance, and musculoskeletal conditions.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
  • Educate patients on injury prevention, wellness, and home exercise programs.
  • Maintain accurate and timely documentation of patient progress and treatment outcomes.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
