About The Position

Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities serving Quakertown, Coopersburg, and surrounding Upper Bucks County communities. Patient opportunities may include Perkasie, Sellersville, Richlandtown, Milford Square, Trumbauersville, and nearby areas. This role offers flexibility, independence, and the opportunity to provide meaningful one-on-one patient care.

Requirements

• Active Pennsylvania Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor patient progress and adjust treatment plans
• Complete progress assessments and discharge planning
• Coordinate care with physicians, nurses, and other team members
• Complete accurate and timely documentation using WellSky
