About The Position

Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) for PRN home health opportunities serving Quakertown, Coopersburg, and surrounding Upper Bucks County communities. Service areas may include Perkasie, Sellersville, Richlandtown, Milford Square, Trumbauersville, and nearby communities. Home health OT is about helping patients regain the skills that make everyday life possible — getting dressed, preparing meals, safely using the bathroom, and navigating their own home.

Requirements

Active Pennsylvania Occupational Therapist License Required
Graduate of an accredited Occupational Therapy program
Valid Driver's License and reliable transportation
Strong clinical assessment and communication skills
Ability to work independently in patients' homes

Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete occupational therapy evaluations in patients' homes
Develop individualized treatment plans
Help patients improve independence with activities of daily living
Provide therapeutic interventions to improve strength, coordination, mobility, and function
Assess home safety and recommend adaptive equipment or modifications
Educate patients and caregivers
Monitor progress and adjust treatment plans
Coordinate care with physicians, nurses, therapists, and other team members
Complete accurate and timely documentation using WellSky
Participate in discharge planning
