About The Position
Sage Group HealthCare Staffing is seeking a Physical Therapist (PT) for a Home Health role in Arbor Vitae, WI. This is a 13-week travel/contract position, open to both local and traveling candidates. The role involves providing Physical Therapy services to home health patients, consulting with physicians and interdisciplinary teams, and developing treatment plans. The goal is to assist patients in restoring function, relieving pain, and preventing disability, allowing them to maintain dignity and independence in their homes. The position requires driving to patient homes daily and timely completion of charting and documentation.
Requirements
- 1-2 years of Physical Therapist experience
- 1 year of Recent/Current Home Health OT experience within the last 3 years
- Home Care Home Base (HCHB) charting experience
- Valid Driver’s License
- Personal Auto Insurance
- PT License for State applying or Compact if applicable
- BLS Certification from AHA or American Red Cross
Responsibilities
- Provide Physical Therapy services for Home Health Patients
- Consult with the physician and an interdisciplinary team in the development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
- Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
- Drive to patient homes in the assigned territories on a daily basis.
- Complete charting and patient documentation in timely manner as per client policies and procedures
