About The Position
Join a growing Medicare-certified Home Health agency on Oahu built around one mission: helping seniors stay steady, mobile, and safe in their own homes. We're looking for licensed Physical Therapists and Physical Therapist Assistants who bring both clinical skill and genuine heart to every visit. This is a per diem position with flexible scheduling, allowing you to set your own caseload. The agency is looking for individuals who can deliver in-home physical therapy visits across Oahu, perform evaluations, reassessments, and fall-risk screenings, and track progress carefully while keeping the care team informed.
Requirements
- Active Hawaii license as PT or PTA
- At least 1 year of clinical experience
- Sharp documentation habits and clear communication
- A valid driver's license and dependable transportation

- Home health experience a plus
