About The Position

We are seeking either a Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our rehabilitation team serving patients in both outpatient and inpatient settings. This is an opportunity to make a meaningful difference while working alongside a collaborative, patient-focused team. The role involves evaluating, treating, and supporting patients in achieving their mobility and rehabilitation goals, developing individualized treatment plans, monitoring progress, educating patients and families, collaborating with physicians and interdisciplinary team members, and completing accurate documentation. The position also includes participation in staff meetings, professional development, and quality improvement initiatives.

Requirements

• Graduate of an accredited Physical Therapy program (for PT).
• Associate degree from an accredited Physical Therapist Assistant program (for PTA).
• Current Physical Therapist license in the practicing state (for PT).
• Current Physical Therapist Assistant license (or eligibility) in the practicing state (for PTA).
• BLS or CPR certification required within 30 days of hire.
• Act 33 Clearance
• Act 34 Clearance
• Act 73 FBI Clearance

Responsibilities

• Evaluate, treat, and support patients in achieving their mobility and rehabilitation goals (within scope of practice).
• Develop or implement individualized treatment plans designed to improve function, reduce pain, and restore independence.
• Monitor patient progress and communicate updates with the rehabilitation team.
• Educate patients and families on treatment plans, recovery, and home exercise programs.
• Collaborate with physicians and interdisciplinary team members to ensure high-quality patient care.
• Complete accurate and timely documentation.
• Participate in staff meetings, professional development, and quality improvement initiatives.
