About The Position

Mobile Health Team Inc. is seeking an experienced Physical Therapist - Outpatient for a 13-week travel assignment in Colebrook, NH. This is a hospital-based outpatient and inpatient acute & swing position at a Critical Access Hospital. The role involves reviewing patient medical histories, diagnosing patients through observation and listening to concerns, developing individualized treatment plans with clear goals and expected outcomes, and managing pain, increasing mobility, and preventing further injury through exercises, stretching, equipment, and hands-on therapy. The therapist will also record patient progress, modify care plans as needed, and educate patients and family members about the recovery process.

Requirements

Must have 2 years current Physical Therapy experience
Outpatient experience is required
Must have and maintain current Basic Life Support (BLS) certification during employment
Strong organizational skills and ability to multitask
Willingness to participate in continuing education and training programs
Strong attention to detail and ability to work independently
Excellent communication skills, professionalism, and reliability
Drug screen and background check (facility required)

Nice To Haves

Inpatient experience is highly preferred

Responsibilities

Reviewing patient medical histories
