About The Position

Community Home Health Care is hiring licensed Physical Therapists and Occupational Therapists for part-time and PRN home visits in the Fort Wayne area. The roles provide one-on-one therapy in clients’ homes, helping them improve movement, perform daily activities more safely, reduce fall risks, and remain as independent as possible. Physical Therapists help clients improve movement, strength, balance, and safety at home. Occupational Therapists help clients participate in everyday activities as safely and independently as possible.

Requirements

Current Indiana license as a Physical Therapist or Occupational Therapist.
Compassionate and dependable, with an interest in one-on-one care.
Strong communication, organization, and documentation skills.
Confidence working independently in clients’ homes and communicating with a care team.

Home health experience.

Responsibilities

Evaluate clients’ mobility, gait, strength, balance, transfers, and fall risk.
Develop treatment plans based on client goals and the home environment.
Provide exercises and functional mobility training.
Teach safe transfers and use of mobility equipment.
Identify home safety concerns and recommend practical changes.
Show clients and caregivers how to support progress between visits.
Evaluate daily activities such as dressing, bathing, grooming, and meal preparation.
Build skills and routines that support greater independence.
Teach energy conservation, safe movement, and adaptive techniques.
Recommend adaptive equipment or home changes to improve safety.
Support upper-body function, coordination, and functional reach.
Educate clients and caregivers on strategies they can use between visits.
