About The Position

Atlantic Health is seeking a full-time Physical Therapist for the dynamic Atlantic Visiting Nurse Dept to work 8am-4pm in Morris County. The ideal candidate will evaluate and treat patients in the patient's home and be a part of a collaborative team of health providers. Atlantic Visiting Nurse provides personalized care for you or a loved one to regain health, stay out of the hospital, and remain living independently at home. Whether or not it is managing life with a chronic illness, living with a disability, returning home from the hospital or rehabilitation facility, or need assistance with end-of-life care, we are available to you in six counties throughout New Jersey: Morris, Somerset, Essex, Sussex, Union, and Warren.

Requirements

  • At least one year of work experience is strongly preferred.

Responsibilities

  • Performs initial examinations and differential diagnosis.
  • Provides and prescribes plans of care consisting of corrective therapeutic exercise.
  • Performs manual treatments consisting of soft tissue techniques, joint mobilizations, and many more.
  • Supervises and interacts with support staff and maintain proper documentation.
  • Evaluates and establishes a plan of care and treats patients accordingly.
  • Documents patient care and charges appropriately, and documents the course of patient care, including progress made and the ongoing need for treatment.
  • Works with other professionals in an effective manner
  • Provides clinically appropriate therapeutic interventions.
  • Evaluates patient response to treatment and provides feedback to the interdisciplinary team.
  • Performs other related duties as assigned.
