About The Position
The Elliot VNA provides 24/7 in-home support and services to patients in Manchester and the surrounding communities. We are committed to helping our patients navigate the challenges they face at home while recovering from surgery or coping with life-limiting illnesses or physical disabilities. The VNA care team provides individualized care across the continuum, bringing peace of mind to patients and their loved ones while providing professional healthcare services in the comfort of home. Provides professional physical therapy services to clients/patients of all ages and families whose needs change from basic to complex and are rapidly changing. Plans and implements age-appropriate specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice.
Requirements
- Degree from an American Physical Therapy Association (APTA) accredited program required.
- Licensed as a Physical Therapist by the state of New Hampshire Office of Licenses Allied Health Professionals required.
- BLS, CPR required.
- Documentation on laptop computer, basic typing skills.
- Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities required.
- Proof of auto mobile insurance required, and minimum coverage met.
- Motor vehicle background report will be conducted.
Nice To Haves
- Minimum one (1) year of experience in health care setting performing similar work activities preferred.
Responsibilities
- Plans, develops, and implements physical therapy care plan appropriately.
- Regularly re-evaluates and updates appropriateness of care plan and physicians’ orders.
- Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved.
- Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes.
- Orients and supervises PTA’s according to State of NH guidelines and VNA policy.
