About The Position

Sage Group HealthCare Staffing is seeking a Physical Therapist for a Home Health role in Alpena, Michigan. This is a 13-week travel/contract position that accepts both local and traveler candidates. The role involves providing Physical Therapy services to patients in their homes, consulting with physicians and interdisciplinary teams, and developing treatment plans to restore function, relieve pain, and prevent disability. The therapist will drive to patient homes in assigned territories daily.

Requirements

• 1-2 years of current Physical Therapist experience.
• 12 months of recent/current Home Health PT experience within the past 3 years.
• Home Care Home Base (HCHB) charting experience.
• Valid Driver’s License and Personal Auto Insurance.
• Must be Covid Vaccinated or provide medical or religious exemption.
• Bachelor of Science degree in Physical Therapy or Similar.
• Current State License as Physical Therapist for MI.
• BLS Certification from AHA or American Red Cross.

Responsibilities

• Provide Physical Therapy services to patients/clients at their homes.
• Consult with the physician and an interdisciplinary team in the development of a plan of treatment.
• Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
• Plans and prepares treatment plan based on evaluation of patient data.
• Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability.
• Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home.
• Drive to patient homes in the assigned territories on daily basis.
