About The Position

This is an exciting leadership opportunity for an experienced, licensed Physical Therapist to lead a brand-new outpatient orthopedic clinic in Elizabethtown, KY, opening in August 2026. As the Clinic Director, you will be responsible for developing clinicians, elevating patient outcomes, and creating a positive, high-performing clinic culture. This role offers a path to multi-site leadership or clinic ownership/equity partnership, allowing you to manage the clinic like your own business while leveraging the support of Upstream Rehabilitation. The position involves leading and mentoring the clinic team, fostering a patient-centered environment, providing direct patient care, overseeing clinic operations, building referral relationships, and partnering with leadership for growth. Continuous professional development in leadership and clinical skills is supported.

Requirements

  • Graduate of an accredited Physical Therapy program (DPT or equivalent)
  • Active Kentucky Physical Therapist license (or eligible)
  • Passion for developing others required

Responsibilities

  • Lead, mentor, and support your team of Physical Therapists and clinical staff
  • Foster a positive, patient-centered environment with high standards for clinical care
  • Provide direct patient care using individualized, evidence-based treatment plans

  • Build and nurture relationships with local referral sources and community partners
