About The Position

Concierge Home Care is hiring a Home Health Physical Therapist Assistant (PTA). The coverage area is Sanford / Seminole County, FL, with territories designed to minimize drive time through clustered scheduling. The role utilizes a pay-per-visit model with potential for high annual earnings, quarterly bonuses, and reimbursement for mileage and data plans. The company provides consistent patient volume and utilizes technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time, allowing clinicians to spend more time with patients. There are also opportunities for growth and clinical support, including structured onboarding, mentorship, preceptorship programs, ongoing training, free CEUs, and career advancement. Employment is contingent upon a successful AHCA Level 2 background screening.

Requirements

• Active, unencumbered Florida Physical Therapist Assistant (PTA) license
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
• Assist patients in improving mobility, strength, and independence
• Collaborate with patients, caregivers, and interdisciplinary teams
• Monitor patient progress and communicate updates to the supervising PT
• Complete documentation using streamlined, AI-assisted tools
