The Physical Therapist Assistant (PTA) will manage an assigned patient caseload daily, ensuring timely care and documentation per the plan of care and physician orders. This role involves collecting health data through home assessments, telephonic contacts, and the Electronic Medical Record (EMR). The PTA will implement care programs including modalities, procedures, exercise, and education as directed by the Physical Therapist (PT). Responsibilities include documenting and executing the plan of care to address patient/family needs, discharge planning, and resource utilization. The PTA will apply clinical reasoning and skills for accurate assessments, evaluate therapy effectiveness, and prioritize patient needs. They will provide and document patient/family education on deficits, goals, techniques, and home programs, including equipment use. The PTA will observe, record, and report treatment reactions or condition changes to the physician/PT and communicate updates on schedules, patient data, and orders with the multidisciplinary team. Incorporating short- and long-term goals into therapy, documented in visit notes, and supporting collaboration with physicians and PT leadership for timely feedback are key aspects of this role. Ensuring clear, timely, and complete documentation per policy and standards, while meeting productivity targets, is essential. The PTA will perform treatments per the State PT Practice Act, Home Health Conditions, JCAHO, Medicare, OASIS, and agency policy. This role also promotes shared governance, decision-making, and research to improve outcomes, maintains current PT knowledge and competence, and integrates evidence and research into practice. Contributing to quality PT practice, communicating effectively in all formats, and collaborating with patients, families, and others in PT practice are expected. The PTA will evaluate their own practice against standards and regulations and accept constructive feedback, using appropriate resources for safe, effective, and cost-conscious PT services.
