The Physical Therapist Assistant (PTA) will manage an assigned patient caseload daily, ensuring timely care and documentation per the plan of care and physician orders. This role involves collecting health data, implementing care programs, documenting and executing plans of care, and applying clinical reasoning for accurate assessments and prioritization of needs. The PTA will also provide and document patient/family education, observe and report treatment reactions or condition changes, and communicate updates with the multidisciplinary team. They will incorporate short- and long-term goals into therapy, support collaboration with physicians and PT leadership, and ensure clear, timely documentation while meeting productivity targets. The PTA must perform treatments per State PT Practice Act, Home Health Conditions, JCAHO, Medicare, OASIS, and agency policy, while promoting shared governance and maintaining current PT knowledge and competence. They will collaborate with patients, families, and others in PT practice, evaluate their own practice, and use appropriate resources for safe, effective, and cost-conscious PT services.
