About The Position

Concept Rehab, a therapist-owned and therapist-led rehabilitation company with over 45 years of experience, is seeking a licensed Physical Therapist Assistant (PTA) to join their team in a Skilled Nursing Facility (SNF) in Avon, Ohio. This role focuses on working with older adults in a collaborative post-acute care environment to enhance their mobility, independence, safety, and overall quality of life. The company emphasizes supporting and valuing its clinicians, offering a culture of clinical excellence for both experienced PTAs and new graduates.

Requirements

Graduate of an accredited Physical Therapist Assistant program
Current PTA license in the State of Ohio
Strong communication and interpersonal skills
Commitment to patient-centered care
Ability to work collaboratively within an interdisciplinary healthcare team

Nice To Haves

Experience in Skilled Nursing, Rehabilitation, Long-Term Care, Post-Acute Care, or Geriatrics
New graduates are encouraged to apply

Responsibilities

Provide physical therapy treatments in accordance with individualized care plans
Help patients improve mobility, strength, balance, endurance, and functional independence
Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team
Collaborate with nursing staff, physicians, families, and caregivers
Participate in care planning and discharge planning activities
Educate patients, families, and facility staff on therapy programs and safety strategies
Maintain timely and accurate electronic documentation and billing records
Assist with restorative and functional maintenance programs
Recommend equipment and treatment interventions that support successful patient outcomes
Support a positive rehabilitation environment focused on quality care and service excellence
