They are seeking a licensed Physical Therapist Assistant (PTA) to join their team in a Skilled Nursing Facility (SNF) in Avon, Ohio. This role focuses on working with older adults in a post-acute care setting to enhance their mobility, independence, safety, and overall quality of life. Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Current PTA license in the State of Ohio
- Strong communication and interpersonal skills
- Commitment to patient-centered care
- Ability to work collaboratively within an interdisciplinary healthcare team
Nice To Haves
- Experience in Skilled Nursing, Rehabilitation, Long-Term Care, Post-Acute Care, or Geriatrics

Responsibilities
- Provide physical therapy treatments in accordance with individualized care plans
- Help patients improve mobility, strength, balance, endurance, and functional independence
- Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team
- Collaborate with nursing staff, physicians, families, and caregivers
- Participate in care planning and discharge planning activities
- Educate patients, families, and facility staff on therapy programs and safety strategies
- Maintain timely and accurate electronic documentation and billing records
- Assist with restorative and functional maintenance programs
- Recommend equipment and treatment interventions that support successful patient outcomes
- Support a positive rehabilitation environment focused on quality care and service excellence
