About The Position
Therapy Partner Solutions (TPS), a therapist-owned and operated company since 2016, is seeking a Licensed Physical Therapist Assistant for home health in Polk County, GA. We are dedicated to providing exceptional therapy services and understand the unique challenges clinicians face. As the provider of home health therapy services for Pruitt Home Healthcare, we are part of a strong, multidisciplinary team focused on collaborative, patient-centered care. We invest in our people through a Clinical Excellence Academy offering mentorship, advanced training, and educational series. Requirements
- Graduate (current or pending) of an accredited physical therapy assistant program.
- Active or pending state physical therapist assistant license.
- New graduates encouraged to apply.

Responsibilities
- Work with a diverse and engaging caseload.
- Provide home health therapy services.
- Collaborate with a multidisciplinary team including physical therapists, occupational therapists, speech therapists, nurses, physicians, and other healthcare professionals.
- Deliver collaborative, coordinated, patient-centered care.
- Complete documentation efficiently and confidently.
