About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, the Home Health Physical Therapist Assistant helps patients regain strength, mobility, and independence in their homes. The role delivers personalized care focused on functional improvement and quality of life. The assistant administers the physical therapy treatment plan developed by a Physical Therapist, provides specified therapeutic exercises and modalities, documents care, and coordinates with the clinical team. The position is patient-facing and requires TB screening. It also requires driving, a valid state driver's license, personal vehicle liability insurance, and reliable transportation.

Requirements

Associate degree in Physical Therapy from an American Physical Therapy Association.
Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
Read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
Apply infection-control concepts and universal precautions when performing patient care activities.
