About The Position

This role is for a Licensed Physical Therapist Assistant (PTA) in the Community-Based Rehabilitation Specialist program at Home Health Care, Inc. The position is full-time with a weekend rotation and is located in the Twin Cities Metro Area. It offers the opportunity to expand clinical skills, enjoy greater independence and flexibility, and work with a diverse patient population including orthopedic, neurological, post-surgical, and medically complex patients. The role emphasizes one-on-one patient care, building relationships, and helping patients remain safely at home. Requirements

• Active Minnesota Licensed Physical Therapist Assistant (PTA) license.
• Passion for helping patients improve function and independence.
• Excellent communication and teamwork skills.
• Valid driver's license and reliable transportation.

Nice To Haves

• Home health experience is welcomed but not required.

Responsibilities

• Deliver physical therapy treatments established by the supervising Physical Therapist.
• Improve patient mobility, strength, balance, coordination, and safety.
• Reinforce home exercise programs and educate patients and caregivers.
• Monitor patient progress and communicate recommendations to the supervising PT.
• Collaborate with nurses, therapists, physicians, and the interdisciplinary care team.
• Complete timely and accurate documentation.
