As a Home Health Physical Therapist Assistant, you'll partner with a supervising Physical Therapist to help patients regain strength, improve mobility, reduce pain, and restore independence—all in the comfort of their own homes. You'll build meaningful relationships while helping patients achieve milestones that change their lives. No crowded clinics. No rushed appointments. No assembly-line therapy. Just personalized, one-on-one care that produces lasting results.
Requirements
- Active Virginia Physical Therapist Assistant (PTA) License Required
- Valid Driver's License and Reliable Transportation
- Strong Communication and Documentation Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Provide skilled physical therapy treatments under the direction of a Physical Therapist
- Implement individualized treatment plans
- Monitor patient progress and communicate findings to the supervising Physical Therapist
- Educate patients and caregivers on therapeutic exercises, mobility techniques, and fall prevention
- Assist patients in improving strength, balance, endurance, and functional mobility
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
