About The Position

Signature Healthcare at Home is seeking a Full Time Physical Therapist Assistant (PTA) to join their home health team. This role will primarily serve patients throughout the east Portland metro area (east of the Willamette River) and offers autonomy, flexibility, and meaningful patient relationships. The PTA will work under the supervision of a licensed Physical Therapist to provide hands-on, one-on-one care to patients in their homes.

Requirements

Licensed Physical Therapist Assistant (PTA) in Oregon
Graduate of an accredited PTA program
Current driver’s license and reliable transportation
Ability to work independently in a field-based setting

Nice To Haves

Home health experience
Familiarity with OASIS and Medicare documentation
Strong time management and clinical skills

Responsibilities

Provide physical therapy treatments according to the plan of care established by the supervising PT
Assist patients in improving: Mobility, Strength, Balance, Functional independence
Observe and report patient responses to treatment
Work closely with Physical Therapists, nurses, and the interdisciplinary care team
Communicate patient progress and recommend adjustments to care plans
Participate in care coordination and case discussions
Instruct patients and families on: Home exercise programs, Safety and fall prevention strategies, Proper use of assistive devices
Complete accurate, timely clinical documentation
Follow Medicare home health guidelines and Oregon state regulations
Ensure compliance with company and regulatory standards
Support patient-centered outcomes and satisfaction
Help reduce hospital readmissions and promote independence
