About The Position

Concept Rehab is hiring a full-time Physical Therapist Assistant (PTA) to provide skilled rehabilitation services in a skilled nursing facility in Port Huron, Michigan. The role supports patients in improving mobility, strength, balance, and independence. Concept Rehab is a therapist-owned and operated rehabilitation company committed to high-quality patient care and supporting therapists’ professional growth. The PTA provides physical therapy treatments under the supervision of a licensed Physical Therapist and in accordance with established plans of care. The position involves working collaboratively with an interdisciplinary healthcare team and promoting patient safety, independence, and functional recovery.

Requirements

Graduate of an accredited Physical Therapist Assistant program.
Current Michigan Physical Therapist Assistant (PTA) license.
Strong clinical, communication, and documentation skills.
Ability to work collaboratively with an interdisciplinary healthcare team.
Compassionate approach to patient care and rehabilitation.

Nice To Haves

Skilled nursing facility (SNF) or long-term care experience preferred.

Responsibilities

Provide physical therapy treatments according to established plans of care and under the supervision of a licensed Physical Therapist.
Assist patients with therapeutic exercises, functional mobility, transfers, balance, strength, and gait training.
Monitor patient progress and communicate changes or concerns to the supervising Physical Therapist.
Complete accurate and timely treatment documentation in accordance with facility and regulatory requirements.
Collaborate with physical therapists, occupational therapists, nurses, and other healthcare professionals to support patient outcomes.
Promote patient safety, independence, and functional recovery throughout the rehabilitation process.
