About The Position

Sage Group HealthCare Staffing is seeking a Physical Therapist Assistant for a permanent placement position. This is a full-time role primarily in an outpatient setting with occasional acute care coverage. The position is open to local candidates and those requiring relocation. The role involves evaluating patients, developing treatment plans, providing therapeutic interventions, educating patients and families, and documenting progress. The Physical Therapist Assistant will also supervise other healthcare professionals and students, recommend adaptive equipment, and participate in team conferences. Maintaining patient confidentiality and adhering to organizational policies are crucial. The position emphasizes quality patient care through teamwork, professionalism, and service excellence. Additional responsibilities include maintaining competency with electronic documentation, promoting process improvement, following safety standards, and managing multiple priorities.

Requirements

  • Physical Therapist (PT) License – Required
  • Bachelor’s Degree – Required
  • BLS Certification – Highly Preferred (within 1 month of hire or transfer)
  • Experience preferred, but open to qualified candidates who meet education and licensure requirements
  • Strong communication skills and ability to collaborate with interdisciplinary teams
  • Ability to work independently with excellent problem-solving, organizational, and time-management skills
  • Ability to demonstrate and teach selected treatment techniques to professional and non-professional individuals
  • Successful completion of orientation and competency requirements
  • Must comply with OSHA standards and infection control requirements
  • Ability to sit, stand, walk, bend, kneel, and perform therapy activities for extended periods.
  • Ability to occasionally lift or carry 21–50 lbs.
  • Ability to frequently perform activities requiring up to 20 lbs. of force.
  • Ability to constantly handle equipment and perform patient care-related movements.
  • Must be able to perform essential job functions with reasonable accommodation.

Responsibilities

  • Evaluate patients’ functional abilities through formal and informal assessments of skeletal, neurological, respiratory, and cardiovascular systems.
  • Develop individualized treatment plans based on patient assessments, goals, and tolerance levels.
  • Create and update plans of care in collaboration with physicians and healthcare team members.
  • Provide therapeutic interventions to restore, maintain, and improve patient mobility and functional independence.
  • Educate patients, families, and caregivers regarding diagnoses, treatment plans, recovery expectations, and home exercise programs.
  • Supervise Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides, and students according to regulatory standards.
  • Recommend adaptive equipment and assist patients with obtaining necessary supportive devices.
  • Document patient progress, treatment responses, and changes in condition according to compliance standards.
  • Complete required documentation for billing and regulatory requirements.
  • Participate in team conferences, interdisciplinary meetings, and quality improvement initiatives.
  • Promote physical therapy services through education and community outreach activities.
  • Function as a case manager for assigned patients and coordinate care with interdisciplinary team members.
  • Maintain confidentiality of patient information and follow all organizational policies and procedures.
  • Demonstrate commitment to quality patient care through teamwork, professionalism, and service excellence.
  • Provide outpatient physical therapy services with occasional acute care coverage.
  • Maintain competency with electronic documentation systems and required technology platforms.
  • Promote process improvement and support program development initiatives.
  • Follow workplace safety standards and properly utilize personal protective equipment when required.
