About The Position

A leading regional healthcare provider is seeking a Travel Physical Therapist for an onsite assignment in Chelsea, Michigan. The therapist will assess patients, develop and update plans of care, and provide physical therapy interventions. Skilled nursing facility experience is required. This is a 90-day contract assignment with a possible extension and one opening. The anticipated start is immediate. The schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m., totaling 40 hours per week. The role includes availability for some holidays and one weekend per month. Orientation to unit policies and systems will be provided. The position works closely with physicians, nurses, and other care team members, with a strong focus on patient safety and accurate documentation.

Requirements

At least 2 years of physical therapy experience.
Skilled nursing facility (SNF) experience.
An active physical therapy license valid in Michigan; this will be confirmed during screening.
Availability to work some holidays and one weekend per month.
Ability to work a full-time day schedule.

Nice To Haves

Prior travel or contract therapy experience.
Experience supervising assistants and students.
Experience with EMR documentation.
Strong references from recent rehabilitation managers.
Flexibility to adapt quickly to a new team.

Responsibilities

Perform initial and ongoing assessments of each patient's condition.
Establish, revise, and evaluate plans of care for identified problems, involving patients and their families.
Provide therapy interventions using standard physical therapy techniques appropriate to each patient's condition.
Oversee physical therapy assistants, students, and other support personnel.
Accurately document evaluations, treatments, and patient progress.
