About The Position

Discovery at Home is seeking a Physical Therapist Assistant in Bonita Springs, FL, to serve the Discovery Oaks Village and Discovery Village Bonita Springs campuses. This position primarily involves working within assisted and independent living communities, with some potential for home visits within the community. The patient population is mainly geriatric, with cases typically falling under Medicare Part A Home Health and some Part B.

Requirements

• Graduation from a Physical Therapist Assistant program.
• State license as a Physical Therapist Assistant (eligible for FL state licensure).
• Valid driver’s license.
• Reliable transportation.
• Current CPR certification.
• Eligible for FL state licensure as a Physical Therapist Assistant.
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Open to candidates who require sponsorship to work in the United States.
• Completion of a Level 2 background screening in accordance with Florida AHCA requirements (contingent upon employment).

Nice To Haves

• Previous experience in home health is preferred.
• PRN candidates will be considered.
• New grad PTAs will be supported and mentored.

Responsibilities

• Serve the Discovery Oaks Village and Discovery Village Bonita Springs campuses.
• Primarily work within assisted and independent living communities.
• Perform some home visits within the community.
• Manage patient cases under Medicare Part A Home Health and Part B.
