About The Position

Exceptional Rehab, located within the warm and welcoming Smyrna facility, is dedicated to delivering high-quality, compassionate therapy services to individuals in need of rehabilitative care. We focus on improving the quality of life for our residents through personalized, attentive care and fostering a supportive, team-oriented environment. We are located in Smyrna, TN. We are seeking a motivated and compassionate Part Time Physical Therapist Assistant (PTA) to join our team. The ideal candidate will be passionate about helping residents achieve their fullest potential while collaborating with a skilled team of therapists and healthcare professionals.

Requirements

  • Licensed PTA in the state the facility is located in
  • A graduate of an approved education course for physical therapy assistants
  • Strong communication and teamwork skills

Nice To Haves

  • Prior long-term care and EMR experience strongly preferred
  • Experience in SNF setting is a plus but not required.

Responsibilities

  • Provide top-notch therapy to residents, helping them improve their quality of life
  • Work with a diverse range of rehab patients and rehab needs
  • Collaborate with a fun and supportive interdisciplinary team
  • Deliver personalized treatment plans in a welcoming, community-centered environment
  • Provides therapy and performs selected physical therapy interventions under the direction and supervision of the Physical Therapist.
  • Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department
  • Obtains data related to the interventions provided.
  • Makes modifications in selected interventions either to progress the resident as directed by the physical therapist or to ensure resident safety and comfort.
  • Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities.
  • Carries out the d/c plan as determined by the PT
  • Maintains proper documentation This includes but is not limited to: creating and maintaining a resident record for each patient treated; completing documentation per policy including timeliness and medical necessity support; assures therapy billing and labor entries are complete, accurate and timely
  • Communicates effectively by maintaining communication with supervisory PT; participating in supervisory visits with the supervising PT as required by the State Practice Act; requesting reexaminations from the supervising PT when a change in resident status is identified; partnering with multi-disciplinary team to carry out plan of care and advocate for resident and communicating with the interdisciplinary team regarding treatments and discharge planning.
  • Relates professionally, positively, and cooperatively with residents, family members, and other employees at all levels
  • Carries out responsibilities in compliance with federal, state, local laws and regulations, and with facility philosophy, policies and procedures
