About The Position

Central Gardens Post Acute is seeking a dedicated PRN Physical Therapist Assistant (PTA) to join their exceptional in-house therapy team. Requirements

  • Current, valid California Physical Therapist Assistant (PTA) License required.

Responsibilities

  • Deliver high-quality physical therapy interventions under the direction and supervision of a licensed Physical Therapist.
  • Provide individualized treatment focused on mobility, strength, balance, functional independence, and quality of life.
  • Monitor resident responses to treatment and communicate progress or concerns to the supervising PT.
  • Maintain accurate and timely clinical documentation.
  • Track resident progress and support achievement of established treatment goals.
  • Follow facility policies, clinical standards, and applicable regulatory requirements.
  • Collaborate with Nursing, Activities, and the broader Therapy team to support comprehensive resident care.
  • Participate in care planning and communicate effectively with interdisciplinary team members.
  • Educate residents and caregivers on safe mobility techniques, exercises, and strategies to promote independence.
  • Support caregivers and staff with carryover of appropriate therapeutic and restorative techniques.
