About The Position

This role involves providing specialized physiatry care in Skilled Nursing Facilities or similar rehabilitation centers. The primary focus is on delivering care to patients experiencing difficulty participating in therapy due to pain or functional limitations. The physiatrist will perform focused assessments, develop individualized treatment plans in collaboration with facility primary care teams, and provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management. This is an autonomous role where the physiatrist operates as the sole physiatrist on-site, supported by a collaborative clinical team for consultation. The role requires coordinating care and referring to other specialties when necessary. It is ideal for a self-motivated and confident clinician passionate about improving rehabilitation outcomes.

Requirements

  • Current, unrestricted PA or NP license.
  • DEA licensed or willing to obtain DEA licensure.
  • Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
  • Genuine passion for providing professional, personalized care in an autonomous environment.
  • Strong communication skills, with the ability to collaborate with multidisciplinary teams.
  • Familiarity with EHR charting.

Nice To Haves

  • Nursing home and/or physiatry experience is a plus, but not required.

Responsibilities

  • Rounding at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location.
  • Delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
  • Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
  • Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
  • Serving as the sole physiatrist on-site, operating with a high level of autonomy.
  • Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.
  • Performing peripheral joint injections (knees, shoulders, hips).
  • Charting in EHR.
