About The Position

This position at Hudson Valley Cardiovascular is responsible for triaging patients over the phone or in the office to ensure prompt identification of patients with high-risk conditions. Information is presented to a physician for required action. The role serves as a liaison between the hospital, office, and referring physicians. The population served is based on the scope of services in the department. Requirements

  • Associates Degree in Nursing minimum, bachelor’s degree preferred.
  • Minimum 2-3 years of experience Nursing Triage or equivalent experience.
  • Knowledge of telephone-based clinical assessment techniques.
  • Knowledge of medical practice telephone triage protocols.
  • Knowledge of appropriate home health information for patients to follow until visit with physician, if they do not need an immediate ER visit.
  • Skill in using electronic medical records to check patient history.
  • Skill in using computerized medical information database during evaluation as guide to appropriate decision.
  • Skill in making triage decisions and responding quickly and calmly in emergency situations such as calling 911 and arranging for ambulance.
  • Skill in IV administration.
  • Valid NYS RN License
  • Valid ACLS Certification
  • Valid BLS Certification

Nice To Haves

  • Cardiology/ICU/ED/OR experience preferred.

Responsibilities

  • Talks directly to patients on the telephone, provides a physician with necessary information and then directs them to emergency rooms or urgent care centers or to see their physician during office hours as instructed by a physician.
  • Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and, as possible, obtaining vital sign and mental status information.
  • Confers with physician.
  • Sends those with high-risk chief complaints such as chest pain to ER immediately as directed by physician.
  • Sets up appointment for patients who do not need to go to ER but need to see a physician or arranges for an appointment scheduler to make the appointment.
  • Process prescription renewals as directed by physician orders.
  • Liaison between referring physicians calling office to speak with Cardiologist regarding mutual patients.
  • Liaison between the hospitals and Cardiologists and disseminate requests for consults or transfers by rounding physicians.
  • Enter these patients into the portal for rounding Cardiologists to see.
  • Participate on the Emergency Response Team.
  • Perform IV Lasix administration.

  • Demonstrates regular, reliable and predictable attendance
  • Performs other duties as required
