Pharmacy Technician - Virtual Care
About The Position
The Virtual Medication History Technician supports Nemours’ Virtual Care Operations Center, located onsite at the Jacksonville Home Office. The role supports inpatient and outpatient virtual care programs by completing medication histories and reconciliation, providing continuous observation for inpatients requiring 1:1 monitoring, managing virtual communication channels, and helping coordinate scheduling, intake, and virtual rooming for home-based and telehealth programs. The position requires attention to detail, prompt responses to patient needs, and collaboration with clinical teams to support pediatric care. The technician conducts medication interviews and reconciliation through virtual platforms, documents in the EHR, identifies and escalates discrepancies, and supports patient education and medication-access workflows within technician scope. Additional duties include operating and troubleshooting monitoring systems, responding to alerts, assisting patients enrolled in virtual programs, maintaining equipment, and supporting the packing and restocking of home remote patient monitoring equipment. The role requires compliance with HIPAA and Nemours standards.
Requirements
- Certified Pharmacy Technician (CPhT) required.
Responsibilities
- Conduct medication history interviews with patients and families by phone or video.
- Reconcile medications at transitions of care, including admission, transfer, and discharge.
- Use the EHR and external sources such as pharmacy fill histories and caregiver reports to create accurate medication lists.
- Identify medication discrepancies, including dose, frequency, duplication, and omissions, and escalate them to a pharmacist or provider.
- Document medication histories and reconciliation activities in the EHR.
- Provide medication education support on adherence, first-dose education, and safe use within the technician’s scope.
- Support prior authorization workflows, refill coordination, and medication access needs.
- Collaborate with pharmacists and providers on virtual pharmacotherapy interventions.
- Assist with high-risk medication monitoring workflows in virtual programs, including ACaH and RPM patients.
- Serve as a Virtual Sitter for inpatients requiring 1:1 monitoring; observe patients in real time and promptly escalate concerns according to Nemours protocols.
- Operate and troubleshoot patient monitoring systems, document accurately, and respond promptly to alerts.
- Answer calls from patients enrolled in virtual home-based programs and provide courteous, timely assistance.
- Assist with scheduling, intake, and virtual rooming for telehealth visits.
- Communicate with clinical teams, escalate concerns through established pathways, and comply with HIPAA and Nemours standards.
- Maintain equipment functionality, report technical issues, and coordinate with IT or biomedical teams as needed.
- Support home remote patient monitoring equipment logistics, including packing equipment for shipment and restocking.
