About The Position
As a Pharmacy Technician, Medication History, you will support medication safety and comprehensive patient care by gathering and accurately documenting patients’ medication histories. You will interview patients or caregivers, access multiple data sources, review current and home medications, and record validated histories in the electronic health record while collaborating with clinical teams. The role requires attention to detail, strong communication, and an empathetic, meticulous approach to medication reconciliation and preventing adverse drug events. Requirements
- High school graduation or GED.
- Three years of pharmacy technician experience in any setting.
- Washington State Pharmacy Technician credential upon hire.
Nice To Haves
- Retail pharmacy experience.
- Graduation from an ASHP-accredited Pharmacy Technician Program upon hire, or an associate degree in another field or equivalent experience upon hire.
Responsibilities
- Collect and document patients’ allergies and current, accurate medication histories.
- Identify prescription and non-prescription medications, herbal products, implantable devices or pumps, and illegal substances during medication history collection.
- Clarify incomplete or inaccurate information with dispensing pharmacies and prescribing providers, using the Controlled Substance Monitoring Database and drug claim databases such as SureScripts and DrFirst.
- Enter validated medication history information into the patient’s electronic medical record or chart according to established procedures.
- Meet or exceed Memorial’s quality control standards for accuracy and productivity based on completed medication histories.
- Consult clinical pharmacists about unresolved issues or matters requiring a pharmacist’s clinical judgment.
- Use software to identify and follow up on missed medication histories or patients with follow-up needs.
