About The Position

Sentara is hiring a PRN/Flexi Pharmacy Technician II for our Outpatient Pharmacy "Meds to Bed" Program at Norfolk General Hospital in Norfolk, VA. The program is designed to help patients transition to home by assisting with medication needs before leaving the hospital, making the transition as simple and convenient as possible. The Pharmacy Tech II acts as a transition specialist, consulting with patients about their recovery needs, discussing medication and self-care requirements, arranging purchases, coordinating payment transactions at the bedside, and ensuring discharge medications and home medical equipment are delivered before leaving. The pharmacy tech performs technician responsibilities in specialized areas of pharmacy practice under the direct supervision of a licensed pharmacist or licensed independent practitioner, demonstrating advanced technical skills, ownership of the work area, and the ability to work independently in defined specialized areas. This role includes training in advanced Specialist Pharmacy Technician job functions, assuming responsibility for assigned tasks, participating in new employee training, filling and delivering medications, ensuring adequate supplies, triaging and escalating issues, using appropriate systems for patient information, and maintaining quality assurance according to departmental and legal requirements. In addition to the above job responsibilities, other duties may be assigned.

Requirements

  • Pharmacy technician license is required in the state of practice.
  • High School Diploma or Equivalent
  • Virginia Pharmacy Tech License (Required)
  • CPhT License (Received through the NHA or PTCB) (Required)
  • 2 Years Pharmacy Experience (Required)

Nice To Haves

  • Transition Care Coordination, Case Management, Discharge Planning, Meds to Beds, Pharmacy to Home experience (Preferred)

Responsibilities

  • Transition specialist that consults with patients about what they will need to have a successful recovery once leaving the hospital.
  • Visit the patient's hospital room to discuss medication needs and self-care requirements.
  • Arrange for purchases and coordinate any payment transactions at the bedside.
  • Ensure discharge medications and home medical equipment can be delivered to the hospital room before leaving.
  • Perform the technician responsibilities in specialized areas of pharmacy practice under the direct supervision of a licensed pharmacist or licensed independent practitioner.
  • Demonstrates advanced technical skills and ownership of the work area.
  • Demonstrates the ability to competently work independently in defined specialized areas of pharmacy practice.
  • Training in an advanced Specialist Pharmacy Technician job functions.
  • Assumes local and/or system responsibility for assigned tasks related to advanced specialist job positions.
  • Participates in the training of new employees.
  • Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner as required by specialty position.
  • Ensures adequate supplies are available for continuous patient care.
  • Triages and escalates any questions or issues outside the pharmacy technician's scope of practice as needed.
  • Uses appropriate systems for processing patient information.
  • Ensures quality assurance is maintained according to departmental and legal requirements.
  • Operates pharmacy equipment appropriately.
