About The Position

Sentara is hiring a full-time Pharmacy Technician II Transition Specialist for our Outpatient Pharmacy "Meds to Bed" Program at Sentara Norfolk General in Norfolk, VA. The program is designed to help patients transition to home by assisting with medication needs before leaving the hospital, making the transition as simple and convenient as possible for patients and their families. The Transition Specialist consults with patients about their medication needs and self-care requirements for a successful recovery. They arrange for purchases, coordinate payment transactions at the bedside, and ensure discharge medications and home medical equipment can be delivered to the hospital room before leaving. The pharmacy tech performs technician responsibilities in specialized areas of pharmacy practice under the direct supervision of a licensed pharmacist or licensed independent practitioner. This role demonstrates advanced technical skills, ownership of the work area, and the ability to work independently in defined specialized areas. It includes training in advanced Specialist Pharmacy Technician job functions and assumes responsibility for assigned tasks. The technician also participates in training new employees, fills and delivers medications, ensures adequate supplies, triages and escalates issues, uses appropriate systems for patient information, and maintains quality assurance according to departmental and legal requirements. In addition to the above job responsibilities, other duties may be assigned.

Requirements

  • High School Diploma or Equivalent
  • Virginia Pharmacy Tech License (Required)
  • CPhT License (Received through the NHA or PTCB) (Required)
  • 2 Years Pharmacy Experience (Required)

Responsibilities

  • Consults with patients about medication needs and self-care requirements for a successful recovery.
  • Arranges for medication and home medical equipment purchases.

  • Performs technician responsibilities in specialized areas of pharmacy practice under the direct supervision of a licensed pharmacist or licensed independent practitioner.

  • Competently works independently in defined specialized areas of pharmacy practice.
  • Assumes local and/or system responsibility for assigned tasks related to advanced specialist job positions.
  • Participates in the training of new employees.
  • Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner as required by specialty position.
  • Ensures adequate supplies are available for continuous patient care.
  • Triages and escalates any questions or issues outside the pharmacy technician's scope of practice as needed.
  • Uses appropriate systems for processing patient information.
  • Ensures quality assurance is maintained according to departmental and legal requirements.
  • Operates pharmacy equipment appropriately.
